ELIGIBILITY:

Applicant must be a full-time doctoral or masters student in one of the ASHA CAA-accredited graduate programs in Oklahoma, majoring in speech-language pathology or audiology.

Undergraduate seniors entering graduate school in the summer or fall of the current year must submit a copy of their graduate school acceptance letter (or a letter from your department chair indicating your acceptance) into one of the graduate programs in the state. (Attach letter within the form below.)

  • Letters must be written by individuals who are familiar with the applicant's academic or clinical work.
  • At least one letter must be written by a current member of OSHA.

Awardees will be notified prior to the fall OSHA Conference.  The manner by which the awardees will be presented the award will be determined based on the current COVID restrictions.
